申請機構/公司必須從以下類別的社會企業採購產品或服務 Organization/Company must purchase from the social enterprises under the below categories

甲類：獲香港社會企業總會《社企認證》的社企 Cat A: Social enterprises endorsed by SEE Mark issued by HKGCSE

乙類：香港社會企業總會基本會員屬下社企項目 Cat B: Social enterprise Project under the Full Member of HKGCSE

丙類：表列於香港社會服務聯會《社企指南》的社會企業 Cat C: Social enterprise listed in “SE Directory” issued by the HK Council of Social Services.

丁類：獲民政事務總署授予「社企Tree嘜標誌」計劃的社會企業 Cat D: Social enterprise issued Tree Mark by he Home Affairs Department of HKSAR

於遞交本申請表格時已完成的採購總金額(採購自獲社企認證的社企，即上述的甲類)，如沒有請填“0“ Accumulated Procurement Amount at the time of application submission ( from SEs with SEE Mark, ie, Cat A SEs ), please enter "0" if not applicable

於遞交本申請表格時已完成的採購總金額(採購自非獲得社企認證的社企，即上述的乙、丙或丁類)，如沒有請填“0“ Accumulated Procurement Amount at the time of application submission ( from SEs without SEE Mark, ie, Cat B, C or D SEs ). please enter "0" if not applicable

申請費用 Application Fee

參加費用為港幣10,000 元正。
The application fee is hk$10,000.
早鳥優惠為$8,000 (於2025年3月31日前遞交申請)
Application submitted on or before 31 March 2025 can enjoy early bird rate at hk$8,000.
請將支票郵寄至「觀塘鴻圖道 16 號志成工業大廈 12 樓 1205 室」，支票抬頭請寫「香港社會企業總會有限公司」或入賬東亞銀行：015-521-40-400151-7, 並將收據電郵(esgaward@sechamber.hk)至本會。
The cheque should be made payable to “HONG KONG GENERAL CHAMBER OF SOCIAL ENTERPRISES LTD” and sent to “Rm 1205, 12/F, Houtex Industrial Building, 16 Hung To Road, Kwun Tong” by post, or bank in to HKBEA: 015-521-40-400151-7, with the bank-in slip’s e-copy attached to email sent to esgaward@sechamber.hk

(5) 本公司明白並同意，凡得獎公司均須委派代表出席頒獎典禮以及配合主辦機構的相關安排; 否則，主辦機構有權取消有關公司的得獎資格。
(6)「ESG商社共創大獎」主辦機構「香港社會企業總會」所作出的一切決定為最終決定。
